How to display incident number and short description on top of the survey

Hi

I want to display the Incident number and short description on top of the survey. Right now, we are showing out of box one. But we need to override that. I looked into assessment_take2 UI page to modify OOB. But no luck.

Can anyone help me on this?

If you look at the code there will be a number coming on the top left corner of the Survey, there you need to look at the UI page and look for the code from where that value is pulling.

Try to create a field on the table & make it display true.
