How to generate report for incident survey form

our incident survey happens on this target : assessment_take2,

can you please help to how to generate a report to see the answers for these survey.

Hello David,

You can create report on the asmt_assessment_instance_question (Assessment instance question table) & use the below filter. Change the metric type to your survey name. Then you will have to group the records via the Instance field.

You can create a report based on the table asmt_metric_result or asmt_assessment_instance_question. This should give you what you're looking for.
