I have created a catalog form where below is one of the variables.
But here the task is I have to first give them a template which the user should be able to download it and fill it and upload here. How can I provide the template file here?

You can directly add the attachment in the variable itself.

If you don't want to use the upload button. You can use the "Rich Text Label" variable and attach the file in the variable. Link the attachment in the rich text field by selecting "Link List". Here is a useful link for this.
