How to setup "survey" for incident and service request.
I have performed below steps:
1. Created new survey from "view surveys" and designed to according to my requirement.
2. Then i have setup the trigger condition as "state is resolved" choosing table "incident.
I am not sure while setting up email notification what needs to be setup in the email notification like:
what should be table?
when to send?
whom to send?

Are you using survey wizards or Assessment Surveys? If you are using Assessment surveys, there is already an OOTB notification which is sent out based on trigger condition.
Please hit correct based on impact of response.

I am using assessment survey but it is not triggering survey for all the incidents even after i set the 100% probability.
please suggest.

Alright, two things to be noted here.
1. Can you verify if you have checked the 'Send Notifications' check box in the survey definition.
2. Trigger conditions on Incident table. Navigate to 'Survey-->Trigger Conditions' and open the Incident table record. Please check the User field is set to Caller or Requester of Incident and filter conditions.
Let me how if the above two things are set perfectly.

I have setup as per below screenshot but even then also survey is not triggered for each and every incident.
