How to take a survey inside of agent workspace

Hello I wanted to see if anyone knew of a way to take a survey inside of agent workspace? Im not able to find a way of doing that through url manipulation. I am able to open the assessment record but not actually take the survey.

 

Any assistance would be appreciated.

Here is the OOB solution I can find. 
If you use Service Operation Workspace, you can enable the 'Workforce Optimization for ITSM' plugin.

Once installed,
- Access your profile by clicking on the 'i' icon next to the user reference field, such as the incident assigned to.  Reference Link - How to access your profile in workspace

-When opening the assessment tab in your profile, you will find the survey option available.. Reference link - How to take a survey in workspace

I tested this in my testing instance, but not sure if the plugin requires additional license.
