The change success score feature is installed after you activate the Change Management - change success score plugin (com.snc.change_management.change_success_score) available with the ITSM Professional subscription only. so Install this Plugin

This can be achieved with OOB Change Management - Change Success Score plugin.

which will add Change Success Score dashboard.

Creation of Change from Incidents ITIL user should be able to create Change record from Incidents. These records should be linked to the Incident. System should create a normal change request and short description should be copied from the Incident." How to perform this functionality.
