I just want to know the table name of "My Assessments and Surveys"

I just want to know the table name which capture survey result(satisfied, dissatisfied, neutral) and with comments.

@SK36 the table name is "asmt_assessment_instance". Once you open the record in this table, the related list will show you all the questions and their respective answers.
