Good Morning !

I want to add to Author name and Knowledge base name on the approval widget.

You will have to clone the existing approval widget and add your customizations. Below link has similar requirement.

After that, you can add the custom widget to the page or keep the existing widget for other requests and add custom widget for knowledge articles.
