Read: 10-tips-for-writing-a-quality-community-question should help you get a meaningful reply in the future.

And maybe go back to the step where you worked on that (looks like a blank form for creating a table record). My guess is that the table fields were not created. See: build_the_needit_application where there is a module on "Tables".

As @Bert_c1 mentioned, you have an empty form with non columns / fields.
