in knowledge article insert button not able to see on the form

I am admin but when i open knowledge article i could not able to see "insert" button. How to enable it?

Compare the UI Actions in prod and Non prod.

Probably an UI action is updated or deleted.

there is no UI action on kb_knowledge table..  but how it is appearing on non prod instances?

can be on parent table also
