In servicenow,I have created flow when it is not working when I submit order guide

I have created a flow like when order guide is submitted,I need to add user to AD group.so when i tested it using test option within flow it is working and adding user to AD group but when i submit order guide it is not working and not adding user to AD group.can someone help me where is the issue?

Hello @ShahidaM ,
You can verify 2 below mentioned points:
1. Ensure Your Trigger condition is Correct for an Order Guide like trigger on "Created on sc_req_item table" and Condition like Requested Item > Cat Item is Your Catalog Item or it trigger is service catalog and flow name added in catalog item of order guide.

2. Check and change the"Run As" Setting from flow properties: By default, it is  "User who initiates the session"  but to resolve permission issue and if you have that approval or rights you can change it to "System User" form flow designer by Clicking on the three-dot menu in the top right corner and select Properties. In the "Run As" dropdown, change the selection and activate and test the flow.
