via attachment enclosed in the catalog item.
Steps for having a template on the form: - Simple way via (attachment enclosed in the catalog) item.
1) enclose the template( format like excel or doc) to the catalog item.
2) Go to description - > write the text you want to write like - you can download the template from this link: Download here
3) Select the section Download here & press ctrl + k
4) Select type as Atachment -> select the attachment -> text as you wish (ss below)
5) save the catalog item
OUTPUT
On the catalog item, you will be seeing like this, on clicking the template will get a download

Hello Madhurya,
If you are using the attachment variable type in the catalog you will be able to enclose only one attachment at a time. For adding more than one file you can use the clip attachment button available on the catalog item.
You can achieve the use case B, In the description create a link from where the user can download the template & after filling it user can enclose the same in the clip button.

We can do it in two ways
1) via link
2) via attachment enclosed in the catalog item.
For now, you can follow the 2nd way, I have mentioned the 5 steps above. For my case, I have enclosed an excel sheet as a template. If you have some other format like a doc that can be also used. Let me know for any questions.
