Sorry, no idea what you are talking about. You already figured out, that there is no table "incident_update". However you want to add an attachment field to it. Makes no sense to me.

when you select multiple incident from list and click on update multiple ,it open the screen incident_update

Ah oke, thats not a table. Thats just a page called "incident_update".
