Including Catalog Variables in Incident Ticket created from Service Catalog.

I would like to include Service Catalog Variables on any incident (heck even change) ticket created through the Service Catalog. Any ideas on how this could be accomplished?

Can't really explain with screenshot but I can elaborate.
We have a Service Catalog item that is supposed to create an incident. We are doing this via Catalog Builder. The Catalog Item has some variables we want to include in the incident, but we are unable to show these variables in the incident itself. If we were creating a Catalog Task, then that would be fine, the variables would be visible, but because we are using Incident, I do not know how to show the Catalog Variables on the Incident Ticket. Does anyone know?

1. From Filter Naviagtor, Search for Record Producer.
2. Click on record producers, then "New"
3. select Incident table and then continue

Please read about record producer, using record producer you can create record with any table, in your case it is incident
