Issue with BR 'Prevent duplicate for account"

Has anyone experienced an issue with account Contact Relationships not being able to be created because of Business Rule 'Prevent duplicate for account"?

This is an OOB Customer Service BR. We have a need to update an existing contact that will allow multiple Contact Relationships so more than one Responsibility Definition can be given to a Contact.

If you've experienced this issue, how would you handle allowing this? Update the OOB BR or is there an alternate such as creating a new Responsibility Access Configuration?

We started having the same problem all of a sudden. We are getting errors contacts. "Duplicate records found with <Account Name>, <Contact Name> combination.
