Need to show checkbox to switch from Additional comments to Comments

Hi, We have a requirement to add a checkbox to switch from Additional Comments and Comments.

Currently we have this on the form it is missing the checkbox below the Additional comments

Based on the links, is it supposed to say this "Show one journal field" or this  "Show All Journal Fields" which is according to this KB0015766

this is appearing on my end, when clicking this nothing happens
