Onhover Following UI Action changes to Unfollow

I have a requirement where I can see a oob UI action of Following and when I hover on it, it changes to Unfollow. So how can  I achieve this functionality.

In which table we can see that UI action, so that we can test it and let you know.

You can open any Incident record and you will find a Follow button.
