Only one user is not able to add attachment

Hi All,
One end user is try to raise one request. In that request attachment is mandatory attachment icon is visible but when they try to add attachment after upload open attachment is not coming. But remaining all end user are able to attach the document.
Please let me know why this is happening.

Bumping for visibility. We also have end users encountering this error. It just continually shows the loading image but never actually adds the attachment to the ticket.
Have tried cache.do, looked into permissions, but at this time it appears its an outside influence preventing them from adding it.
Has anyone found a solution or a better way to troubleshoot this error?

Hello Sirri,
check the length of the user_name field if it's more than 40 characters so if the case then this is the issue, you must back to the OOTB length (40 caracteres).
