I wanted to let anyone else that might be looking for the answer what I found during my research on this very topic.
There is a UX Forms Action Layout Group for "Create" for the interaction table which contains a list collector of several actions (Create incident, Create request, Create change, Create problem):
Removing the actions, Create request and Create change, should resolve this issue.
