Report to show Incident survey responses with assignment group

-just need the list of records with INC number and assignment group

We have OOO survey  for incident table.

Report on "asmt_assessment_instance" table to report on survey (incident).

You can see the below Snap for fields available on "asmt_assessment_instance" table.

On the report give the condition for Metric Type.

You can just dot-walk as shown above through the report designer for requested item to show the list of requested item and survey responses with assignment group.

According to your second question, i don't fully get what you're trying to accomplish . can you please explain some further details regarding: "- show assignment group of all catalog task as one record per survey response".
