REQs not linked to RITMs

I've got a couple of REQs that aren't linked to their RITMs via the related list, and also aren't showing an attached workflow:
By contrast, this is what a normal REQ looks like:
However, the related RITMs DO exist, and they link back to the REQs:
The two problem REQs were created a few days apart, and there have been many other REQs created from the same request form since that don't have the problem. Anyone have any idea what might be going on?

How was the Request created?
is created by submitting a form or through a script?
if it's through the script and the script is simply inserting the record in to the sc_request table this doesn't work
if you are submitting a form and not working that's an issue

Things to check
1) did someone create that REQ using new button
2) did someone created that REQ via script
3) any scheduled job or flow you checked
