Risk Assesment is not working as OOB

On the normal change/emergency change  After filling all mandatory fields and clicking on risk assessment in the Related link always risk field setting to high.

But when change form saved and state is new and when we click on risk assessment then risk calculations are working correctly as OOB.

Can anyone help me why this is happening.

The risk level is determined based on your assessment responses and is calculated accordingly. Try filling in the mandatory fields, save the record, and then complete the risk assessment.

Additionally, please re-enable the Risk Assessment plugin, as the issue might be related to the plugin itself.

i have already try in this way but not working

Did you repair the plugin as well?
