RITM mandatory fields are losing its values once form submitted

Hi All,
I'm facing a strange issue in my production instance.
We do have a Catalog Item - 'DB Access Request' form. It has few mandatory fields. In some cases, submitted form does not have values on the mandatory fields.
Due to this, Approvers are not able to approve the request. This is not happening every time, but in rare cases. Even we have tried to reproduce the issue but no luck.
Also one more thing is, only the text fields are missing its value & not the lookup fields.

Can any one help me on this. I'm struggling with this issue for more than 2 days.

Hello,
For the next persons who will have this issue :
You should definitely check the Catalog UI Policy Action to be sure that the checkbox "Clear the variable value" is false (not checked).
