Service Portal header menu shows "Page not found" error

Hi everyone,
I inserted a feedback widget on the header of my portal so it will show up on every page on the portal. I did this by adding a Server script and HTML in the Header Menu widget I copied. However after doing this the header buttons are showing an error on the portal.
How would I troubleshoot this problem?

Check in the HTML if you have closed all the DIV.

Hi Chris,
Assuming your feedback widget is customized, please check in your custom widget. Use the preview for test run of the widget. If it shows the error in the preview then you need to change your feedback widget acordingly.

The feedback widget is available in ServiceNow. The only customization I have done is only show one feedback button show on a page. (There are 4 options). The error occurs in the preview, what needs to be done to get rid of those errors in the second picture?

Hi Chris,
You mean to say you have cloned the widget and then amended as per your need. Can you please share the screenshot of the code or if you can share the XML of both of your widgets, so that it can be found out?
