Service Portal header menu shows "Page not found" error

Hi everyone,

I inserted a feedback widget on the header of my portal so it will show up on every page on the portal. I did this by adding a Server script and HTML in the Header Menu widget I copied. However after doing this the header buttons are showing an error on the portal. 

How would I troubleshoot this problem?

Hi Manan,

I was able to add the feedback widget to the footer and get the result that I wanted. I think this is enough for what I am trying to accomplish right now. Thank you for your help!
