ServiceNow - How to see what's been modified from OOTB?

Hello,
I have a question that I haven't been able to find the answer or any similar questions to it. I'm wondering about how you can determine what's changed or different from the OOTB installation? We have had a few cases where our instance was experiencing different behavior than the OOTB behavior. How can we tell what's changed in our instance? How does SN update these files? If we modify any SN script, rule, etc. does that mean that it will never get updated from SN? Can I diff my instance vs an OOTB instance?
Any help would be appreciated.

A little more information - We've had ServiceNow in the organization for a number of years. Over these years, people have come and gone, but the changes they made have stayed. I'm sure some of the changes were to fix things that were possibly broken or not implemented in SN at the time, but I would assume a number of these should be reverted. I'm trying to determine how "bad" the situation is and the easiest way to remedy it. I wish it was just as easy as getting a new PDI and applying a couple of update sets to it.

I take it that by the lack of responses that this is a MAJOR problem for ServiceNow and NO ONE wants to talk about it. I guess we will all continue to ignore this situation and act like it's not happening and causing problems for everyone. Good times.
