Showing error message "An error has occured" when i load the case form in portal

Hi all, 

I am facing an issue in the ESC portal. There are couple of record producers which will create case in custom scope. Now, if I create a case in the portal and search with the case number in the global search it returns the case and I am able to see the case details in Form (form-widget OOB). 

Issue: When there are variables added in the case then it throws an error - "An error has occurred". If I check for any case without variables (cases created via email) then there isn't any error message. Attached screenshots.

What I found: When I right-click and inspect the page I found an issue and it's showing the error message as scripted in a JS script. Attaching the screenshots for reference. In the console it says: "Was not able to find parent widget". 
I also tried to find the source code in table sys_js_include but no luck.

I would appreciate any help/suggestions on how to remediate this issue.
