Submit button is freezing on incident form in Service Portal (London)

Hi Community,

 

I am facing a issue when ever I'm trying to request a incident from service portal (Incident record producers) the screen is getting Freeze when I hit submit and I'm unable to request an incident. How to overcome with this issue.
