Survey Title

Hi

I need to display the Short Description of the REQ while the survey is being triggered.

We are using this IT Customer Satisfaction Survey.

When REQ and INC is closed complete, the survey should trigger

This is happening.

But when Incident is being triggered, the Survey includes the Short Description.

When REQ's survey is triggered it is not displaying the Short Description of REQ.

How can we achieve this??
