Toggle button on service portal

HI All,

As shown in the above screenshot, I need an Enable notification check box UI similar to Accessibility enabled toggle button. How to get the same UI on my check box. What changes need to be made on my HTML code, below is the HTML.

Below is the html used in my widget:
