UI Actions for one custom workspace have disappeared after upgrade to Xanadu

When upgrading from Washington to Xanadu, one workspace's UI Actions have disappeared. All the UI Actions appear in the ui_action table, they are all marked to formulate in Workspace and all are in the declarative action table. I have checked the upgrade logs as well to see if there was anything that changed and needed attention for UI Actions but nothing. I upgraded UI Builder to the latest version to see if that was the issue but that is not - even the OOTB Save is not appearing. Two new UI Actions have appeared which is Compose Email and Request Info that should not be a part of the workspace UI Actions and the UI Actions appear in the Classic UI version and work fine.
This table extends from case management and does have 2 related child tables that extend from task and this just started happening. This app has been around for the last 2 years and no issues at all until now.
They are working in our test environment which we have not upgraded yet but need to upgrade soon.
Note: I did put this ticket in HI but because there are custom fields with a custom app, they may not be able to help so much. Any other idea's on how to troubleshoot?

hi yes! so my parent table was extended from the case management table instead of task, and there were changes that happened with the release for case management. The suggestion was to create a record in the "sys_ux_form_action_layout" and then it should automatically map your UI Actions. Let me know if you need any help!
