Universal Request not created for Request's having multiple RITM's

Universal Request deployed on our instance. For every single Incident, HR Case or RITM, an UR record get's created on universal_request table.
We have a catalog item for whose service request (REQ), 2 different requested items (RITM) get's created. For these RITM's, universal request is not created.
kindly help me on this.
