Using ATF i need to achieve particular serivice request from an incident submitted

I need to achieve using ATF the below scanrio 

 

A incident is created and from that incident i need to create a particular Service request from both native UI and Workspace

 

Any suggestion

 

Because from Workspace UI i was able to search the catalog item and open the catalog item but i an not able to fill the values

 

any suggestion
