We did not receive knowledge articles in approval queue

Hello Everyone,
The knowledge article author have edited 'Introduction' field in one article. That edit I can see in the history, but in the same history there is no update recorded for checkout and publish. But the article is in published state, then how he can able to edit article without checking out.
And the author modification recorded in 'article versions' as well, but there is no approval triggered for managers. How this is possible, can anyone help to debug this issue. Screenshots attached.
After workflow is published, the only edit we can see for 'introduction' field. But how without checking out and how the article is in published state without managers approvals.
