I want to define an approval action in FlowDesigner.
Do I need to have a user as a manager who can do the approval?
I don't know what a manager is.
Also, what role should the user who can do the approval have?

approve_admin lets you approve any approval record
approver_user allows you to approve as yourself only

You need to speak to your account manager as to who is entitled to approve.
Generally speaking, the approver_user role is required to approve in Service Portal or mobile in an OOB system. Approver or Business Stakeholder license grants this entitlement. You need to check with your account manager to see if approval via email is allowed without the role - generally the answer is no.

ServiceNow provides the roles "approve_admin" and "approver_user", how do we use the two differently?

I'll add that it is generally accepted that itil can approve, however, OOB configuration won't consider the itil role as being an approver in Service Portal or mobile.
