Discuss button is newly added as part of Utah release. It work to start a chat with user with in ServiceNow. It is SN internal chat messenger.

When I click on post to activity stream it does not work or do anything is it supposed to save the discussion to the work notes and additional comments?

Agree, I noticed this 2 days back during my training. It seems a bug or some settings need to do.

Created a ticket to HI Support will chase this
