What triggered this message?

The following message appeared when a standard change request was created. Could someone please advise me on how to trace what caused this message to appear?  Thank you

Looks like on-load client script..

@JessicaM6884345 The message could come from an onLoad client script on the change request form or a UI Policy script which runs on the change_request form.
