Workflow not being attached to Catalog Item

Hi,
I created a Catalog Item and attached a workflow to it in Dev instance and captured it in the Update Set.
But when I when I moved it to QA instance, the workflow was added but it wasn't attached to the Catalog Item, so I added it manually and captured it in update set as well.
Then when I moved it to Prod the same thing happened, the workflow didn't get attached to the catalog item. So I added it manually. It worked fine for a while but later it automatically went back to the way it was i.e not attached and it was taking up the default workflow.
Please help.

Also, make sure if any Business Rule is running on RITM table which is making this scenario to happen.

Hi, Can you check if someone is updating the workflow manually? or try to see any scripts that is causing this issue
