Workspace Record page shows empty form

I'm having an issue trying to display a form within the Service Operations Workspace. There are certain form views that will not load the fields in the form. I've tried creating new form views under the baseline Service Operations Workspace (sow) view, but it will not display. It does load correctly within the Agent Workspace. 

 

Please help.

I am facing same issue. I also noticed that when I hover over the user profile icon (with initials UU), result is Unknow User / Available
