I am trying to get my menu to display like this image, but it seems to stop at level 2 and will not show anything under it. How do I configure my taxonomy/APN to show level 3?

Does your menu starts with the Topic or Navigation Menu Item ?
If it starts with the Topic you will only see two more levels of topic. I have attached the documentation link below and you can go through "Useful Tips and information" section.
