Guided tour help

Hi Team , 

In a portal we have widget called Report a problem , when we click over this widget it's lead to a form , i want to create guided tour on that form .

can someone please help me with it ?

Create a new tour and then open the tour outside or tour designer. There is an additional url parameters field. Add sys_id=[Sys ID of the form]. Then you can open the tour in tour designer and be on the form. Example screenshot below.

Thanks Brian to guide me through the process, however i have created guided tour but looks like not working , 

what I am missing out ?

also I noticed in tour section there are not showing any created tour
