Portal page not rendering correctly

Hi All,

Requirement is to populate form details on portal page as it is displayed in OOB functionality. We had to map value of a reference field variable (that is in a variable set) to Opened For and Subject Person fields on portal, as shown in image.

We are able to set the variable value in the mentioned fields but that data is visible only on case not on portal. We are able to set value via producer script but on portal, page is not rendering correctly. 

We want it to display like this:

But currently, after submission, this is the portal view:

Please suggest some ways to achieve it.
