Take Questionnaire widget not loading on Service portal

Hi,
When users are trying to click on Take Questionnaire widget (Emergency Self Report) after submitting their Covid-19 status. The widget page returns a blank page. Strangely this widget only works in DEV. We have compared all our widget configuration with DEV and PROD and found no difference in versions. We also checked if any ACL or widget role is impacting, and it looks good. Kindly help determining the possible root cause. [Refer to Screenshots attached]

Did you find a solution? I have the same issue, one day it was working in all instances, the next day it was broken in dev and has been that way for 14 days.
