Appointment Booking in FSM

Hi Community,
I have created appointment booking by adding template to it in catalog item. But, when I create appointment booking from portal, in work order the template has not been applied in work order.
Could anyone help me out of this?

I needed that why the template is not been applied even though I have given while configuring appointment booking. Looking through what you have sent is for creating booking from portal and error while inserting catalog item.

Are you getting any specific error /error message. If yes, Can you share that.

There is no error message, just template is not being applied in WO when created booking from portal.
