The widget is "Standard Ticket Header" , You don't need to modify the widget to do that. 

 

The Standard Ticket page is controlled by the Standard Ticket Configuration ( ticket_configuration )

 

In the filter Navigator -> ticket_configuration.LIST

So, for incident, go to Standard Ticket Configuration, select Incident and add your fields under "info fields", it will reflect
