submitted advanced risk Assessment can not be approved

Dear IRM Experts, 

We are experiencing an issue with Advanced Risk Assessments in ServiceNow.

Once an Advanced Risk Assessment is submitted, it moves to the state “Waiting for Approval.” However, it cannot be approved because no approval task is generated. Additionally, the Approvers related list remains empty.

We have tested this with several different risk assessment methodologies and multiple approval configurations, but the issue persists in every case.

As a result, after submission there is no approval record created and no approver assigned, which prevents the assessment from moving forward in the approval process.

Has anyone experienced a similar issue or knows what configuration or condition might be causing approvals not to be generated?

Any suggestions or guidance would be greatly appreciated.
