Automatic email generation in ServiceNow for catalog task

We have a "Phone number" catalog task in which we have a variable 'Action' which contains options like: Add New, Modify, & Delete. The User and line manager need to be informed about the new phone number, the idea is to create and send an automatic email as soon as the task is completed and the number is added to the task.

 

What condition should be added here ?

Please help.

If state changes to complete, send email notification. Those are the conditions you should add, but you are already mentioning them yourself, so I'm not sure what you exact question is.

create a notification based on the above conditions and add all the above conditions. You can get field values in the notification.
