Playbook card status are getting cancelled automatically

Hi Team,

I have build a generic playboook cases on case table.

where while testing after first step, all other steps are getting cancelled (instead of IN progress) as below:

Please help me, if anything misconfigured!

Could you please verify whether all actions created within the Flow Designer,whether they are published should not be in draft satte,  specifically those involving the addition of a custom or out-of-the-box flow, result in the cancellation of the card activity?
