Add a custom field to the "Accept Resolution" TO-DO page in Employee Center

Hello! I'm trying to add a custom field to the "Accept Resolution" TO-DO page in ESC. See screenshot to see the page.

How do I edit this page to add the new custom field? Which table should I add the new field to? Requirement is to prompt user to select a reason for "Not Accepting" the resolution if they click the No button in this page . I'm not sure where the Accept/Reject input is stored except that the State changes to Closed Complete if accepted or it reverts to Work In Progress on Rejection.

It's a UI page which is opened on click of that button

So you will have to add new field on that UI page
