Collect Employee Input - Employee Forms

Hello Everyone.

I've been working in a LifeCycle event in which an HR Task is assigned to a manager, I associated the survey created in the employee form in the HR Task (Collect Employee input), but the survey is not being displayed in the To Do's.

Does anyone know what can be happening?

The task is assigned to the correct user in blank

If I put a comment in Activity:

The survey now appears in the task that was in blank

Additional to that, the survey created in form designer is not being shown as in the preview (dropdown):
